We are a large PCN of 11 practices and over 103,000 patients. This is an opportunity to join our dynamic team of 12 pharmacists and 3 technicians. The role will be practice‑based, so we would not consider applications from individuals looking for a remote position. This is a really exciting opportunity to join our PCN as an organisation that employs the majority of our Pharmacists and Technicians directly, which enables us to contribute to and support their learning and development under the leadership of an experienced Lead Clinical Pharmacist.

Main duties of the job

  • To participate in the provision of a Clinical Pharmacy Service to our member GP practices;
  • Provision of routine clinical medicines reviews;
  • To review and reduce where possible inappropriate poly‑pharmacy and wasteful prescribing through timely and effective clinical medication reviews;
  • Reconcile patients' medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge;
  • Interface with community and hospital pharmacy colleagues and develop referral processes between primary care professionals, including the promotion of the repeat dispensing service;
  • Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates;
  • Manage patients and healthcare professionals' medicine queries;
  • Work with primary care professionals and patients to implement NICE and other evidence‑based guidelines;
  • Run clinics where appropriate to meet the needs of the Primary Care Network;
  • Contribute to multi‑morbidty reviews and multidisciplinary reviews;
  • Provide medicines information and training to Primary Care Network healthcare professionals and admin staff;
  • Act as a source of medicines information for all of the practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives e.g. around out of stocks).

We are a large and diverse PCN covering Nuneaton and Bedworth in North Warwickshire, and currently have in excess of 60 ARRS staff working across our locality incorporating Care Coordinators, Social Prescribers, Health & Wellbeing Coaches, Mental Health Practitioners, First Contact Physiotherapists, Dietitians, Physician Associates and Nurse Associates alongside our Clinical Pharmacists and Pharmacy Technicians. We have a dynamic Senior Management Team supported by an established Digital and Transformation Team. This has allowed us to implement a number of initiatives including a telephone triage appointment system (Rapid Health), a service to support patients experiencing poor sleep (Sleep Station), pathways for patients struggling to manage long‑term pain conditions (Pure Health & Wellbeing), monthly Pain Cafes and menopause drop‑in clinics, and a 12‑week programme run in conjunction with Sky Blues in the community to support patients with their Health and Wellbeing.

We have an established and respected team of Pharmacists and Pharmacy Technicians working across our 11 practices, all at different places in their career journey – from recently qualified to experienced prescribers. The team is led by an experienced Lead PCN Clinical Pharmacist who is part of our Senior Management Team. We are an organisation that values their employees and are keen to support staff training and development. The majority of our Pharmacists only work across 2 member practices.

Job responsibilities

  • Working within the Primary Care Network team to undertake medication reviews particularly in high risk groups such as:
  • Poly‑pharmacy
  • Renal impairment
  • Hepatic impairment
  • Substance misuse
  • Patients on high‑risk medicines
  • Improve prescribing practice through educational support for all prescribers within the Primary Care Network.
  • Lead on where changes in evidence require changes in prescribing across patient population e.g. where a drug is withdrawn or indication changes.
  • Liaise with colleagues in community pharmacy to align support for medicines adherence.
  • Support improvements in clinical care through practice‑based audit and implementing change.
  • Prescribing advice to prescribers in the Primary Care Network e.g. temporary non‑availability of drugs.
  • Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines.
  • Provide advice and support for patients with self‑limiting conditions.
  • Encourage cost‑effective prescribing within the PCN member practices.
  • Review medications for newly registered patients.
  • Implement and embed a robust repeat prescribing system and electronic repeat dispensing.
  • Re‑authorise medication for repeat prescribing within scope of practice.
  • Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required.
  • Support clinicians with the management of patients suffering from drug and alcohol dependencies.
  • Actively signpost patients to the correct healthcare professional.
  • Assist with QOF targets, including running LTC review clinics in individual or group settings.
  • Administer influenza and any other required vaccines when requested to do so.
  • Provide education on medicines for the clinical team.
  • Review the latest guidance ensuring the PCN conforms to NICE, CQC etc.
  • Act as the PCNs point of contact for all medicines‑related matters, establishing positive working relationships.
  • Person Specification

    Qualifications

    • Completion of an undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council.
    • Minimum of 2 years post‑graduate experience in pharmacy, with at least 1 year in general practice.
    • Member of the Royal Pharmaceutical Society (RPS).
    • May hold or be working towards a postgraduate pharmacy qualification and has an awareness of the breadth of common acute and long‑term conditions that are likely to be seen in a general medical practice.
    • Member of or working towards (through foundation) RPS Faculty membership.

    Other

    • Holds a full driving licence and owns a car, as there is a requirement to travel between practices.
    • Knowledge of EMIS.

    Experience

    • Relevant experience of service delivery within the NHS and PCNs.
    • Excellent influencing and relationship skills with colleagues.
    • Ability to work as a part of a multi‑disciplinary team environment.
    • Ability to engage effectively, commanding respect from a wide range of stakeholders including clinicians, patients and the public and in particular, ensuring effective two‑way communication within the Primary Care Network member practices.
    • Experience of supervising staff and involving and empowering a range of clinical professionals to make healthcare services more cost‑effective and better quality.
