Job overview:

Do you want to help patients remain living independently in their own homes, working within a multi‑disciplinary team? If so, we have an exciting opportunity for a dynamic registered specialist Nurse or Physiotherapist to join our highly specialised respiratory clinicians, providing care to patients with respiratory disease across North of Tees. The post holder will be responsible for providing high standard of care to patients across Hartlepool and Stockton, caring for respiratory health in a clinic and home environment. Driving access and a valid full driving licence are essential.

Main duties of the job:

  • Pro‑active management of patients requiring immediate specialist and comprehensive assessment and pathway‑based interventions.
  • Direct links into the hospital and full support from a wider team, including consultants.
  • Assess and treat patients with complex presentations or multiple pathologies, using advanced assessment techniques and adapting care plans/treatment programmes accordingly.
  • Work flexibly, including weekend working; both full and part‑time hours will be considered.

Detailed responsibilities:

  • Work collaboratively with Specialist Respiratory Practitioners to support and care for patients in the community and patient setting.
  • Rotate into other clinical roles as part of an innovative, responsive multidisciplinary team.
  • Deliver high‑quality care in the community setting, including Asthma, Pulmonary Rehabilitation and home oxygen service.
  • Provide care via the “hospital at home” service, delivering complex and intensive support in patients’ homes for COPD and Bronchiectasis exacerbation management.
  • Expand the service to other respiratory conditions and support early discharge from hospital.
  • Assess patients and provide treatment plans; use PGDs within the service for timely treatment.

Person specification:

  • Qualification: NMC or HCPC registration
  • NMP Respiratory experience
  • Experience working within respiratory care with varied client groups
  • Experience in community working
  • Able to work autonomously
  • Evidence of working autonomously
