Senior Practitioner Band 6 role working in the Crisis Resolution Home Treatment Team. Within this role you will carry out emergency assessments from Community Teams, GP's and A&E Out of Hours in view of gate-keeping possible admissions, ongoing care or referral to external teams. The team also visits Service Users at home to plan and implement care packages for Crisis and Home Treatment Team patients.
