Workstyles: Your workstyle in this role will require you to work from a fixed workplace. The role has become available to join a committed and motivated Patient Flow Team who work hard to deliver excellent standards in a positive working environment. The Hospital Team works in partnership with individuals to identify what matters to them. This includes undertaking Mental Capacity Assessments, Continuing Health Care Assessments, leading Best Interest Meetings to ensure people have a safe and timely discharge from hospital. We work alongside a range of professionals within the teams, including colleagues from the health board, therapies and external providers.

It is an exciting time to join adult social care with friendly, experienced and supportive colleagues who are dedicated to providing a service that puts citizens at the centre of our work.

Ideally you will have:

  • Applicants will need to be social work qualified and professionally registered as a social worker with Social Care Wales.
  • You will need a knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, to include outcome focussed assessment and care planning, knowledge and working practice of Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
