The Community Mental Health Team based at The Forge (Port Talbot) are looking for a full‐time Social Worker with community mental health and ideally AMPH experience, alongside a rounded knowledge of current best practice in mental health.
The CMHT is a well‐established multi‐disciplinary team comprising mental health practitioners from both NPT Council and Swansea Bay University Health Board, working closely with primary care, Police, the Health Board, Drugs and Alcohol Services, and the Crisis Resolution and Home Treatment team.
You will undertake assessments, reviews and care planning, and deliver a high quality service to our service users, working within the legislative framework of the Mental Health Act 1983 (amended 2007) and the Mental Health Measure (Wales) 2010.
The role requires a sound value base, excellent communication skills, and a strengths‐based, outcome‐focused approach. This position will be based within the Community Mental Health Team at The Forge, with hybrid working arrangements.

Ideal qualifications:
- Social work qualified and professionally registered with Social Care Wales.
- Substantial post‐qualifying experience, with at least 6 months in a mental health service.
- Experience as an Approved Mental Health Professional is desirable.
- Knowledge of the Social Services and Wellbeing (Wales) Act 2014, outcome‐focused assessment, and working knowledge of the Mental Health Measures (2010), Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
