Neath Port Talbot Council is seeking two Community Connectors to join Fy Mywyd Independent Living Service. You will provide 1:1 and small-group support to adults with learning disabilities or neurodivergent conditions to build independence, access community opportunities and develop valued relationships.
You will plan, deliver and evaluate activities, support independent travel and money handling, maintain accurate records and raise safeguarding concerns promptly.
