About the Role

A highly rewarding opportunity has arisen for a compassionate and skilled Independent Living Assessor (ILA) to support vulnerable adults in maintaining independence and safety within their own homes. The role focuses on enabling service users to achieve greater autonomy, reduce dependence on formal care services, and improve overall quality of life.

Key Responsibilities

  • Undertake functional assessments to evaluate individuals' ability to complete daily living activities safely and independently.

  • Coordinate and monitor reablement programmes, setting goals and reviewing progress regularly.

  • Manage a caseload efficiently, with weekly allocations and timely support planning.

  • Arrange and install appropriate equipment and recommend minor or simple major adaptations to enhance independence.

  • Support manual handling and transfers in collaboration with Occupational Therapists and Physiotherapists.

  • Develop outcome-focused support plans tailored to individual needs and aspirations.

  • Process referrals and discharge assessments, identifying ongoing needs and assessing eligibility in line with the Care Act 2014.

  • Work closely with multidisciplinary professionals and external agencies to ensure coordinated service provision.

  • Encourage the use of assistive technology to promote self-reliance and reduce long-term service use.

  • Maintain comprehensive and accurate records; contribute to performance, safeguarding, and service quality monitoring.
