Job Purpose

We are seeking a dedicated and compassionate Leaving Care Personal Advisor to join the Children's Social Care Service in Sefton. The successful candidate will play a pivotal role in safeguarding and promoting the welfare of young people who are preparing to leave, or have recently left, local authority care. This post offers the opportunity to make a genuine difference - supporting care‑experienced young people to achieve independence, stability, and positive life outcomes.

Key Responsibilities

  • Provide advice, guidance, and practical support to care-experienced young people.
  • Take part in the assessment, preparation, monitoring, and review of Pathway Plans, ensuring a person‑centred and empowering approach.
  • Coordinate and facilitate access to appropriate services and opportunities.
  • Maintain accurate and up‑to‑date records on ICT and other electronic systems.
  • Promote positive health, well‑being, and confidence in young people, supporting them to reach their full potential.
  • Identify, assess, and report risks in accordance with safeguarding legislation and local procedures.
  • Work collaboratively across agencies as part of the wider corporate parenting team.
  • Attend relevant meetings and represent the department where required.
  • Contribute to service development through team participation and professional learning.

Requirements

  • Experience working with young people, ideally within a social care, housing, or youth support setting.
  • Strong understanding of safeguarding principles and legislation.
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport.
  • Competent in maintaining case records and managing caseloads effectively.
  • Eligible to work in the UK.
