Personal Advisor β Leaving Care Service

Are you passionate about supporting young people as they transition into independent living? We are seeking a dedicated Personal Advisor to join a well-established Leaving Care Service, delivering vital support to care leavers aged 18-25.
You will play a key role in ensuring young people are provided with appropriate accommodation, access to education, training and employment, and the life skills they need to thrive.
Key Responsibilities
- Build strong, respectful relationships with care leavers and their support networks.
- Undertake and review Pathway Plans to assess needs and track progress.
- Work collaboratively with multi-agency professionals to ensure cohesive support.
- Provide practical assistance to promote independence and personal development.
- Monitor accommodation and welfare, escalating concerns where necessary.
- Maintain accurate records and contribute to assessments and risk management.
- Participate in multi-agency meetings and care reviews.
- Work flexibly to meet the needs of young people, including occasional evenings or weekends.
About You
- Hold an NVQ Level 3 (or equivalent) in a child, youth or family-related field.
- Have substantial experience supporting young people or families in a social care, housing, or community setting.
- Demonstrate an understanding of child development, adolescence, and risk management.
- Possess excellent communication, problem-solving, and report-writing skills.
- Able to work independently and as part of a collaborative team.
- Confident in using IT systems for record-keeping and case management.
