Live Well Wakefield Social Prescriber - Trailblazer

Are you passionate about empowering people to live happier, healthier lives? Want to be part of a multi award-winning service that’s making real change? This is your chance.

We’re hiring a Social Prescriber to join the Live Well Wakefield team, a community-based service that supports adults aged 18+ to improve their health, wellbeing and independence. This role is part of our Trailblazer work focused on supporting individuals around employment.

We welcome applications from individuals interested in applying for the role as a new employment, as well as colleagues who may wish to undertake the role on a secondment basis – gaining understanding of the district's largest social prescribing programme and how to create links with your own organisation.

We are happy to have conversations with VCSE organisations about secondment arrangements that provide opportunities for staff development, while bringing valuable experience and expertise into the Live Well Wakefield Team. Contact Natalie Jones, Head of Operations, to discuss: natalie.jones@nova-wd.org.uk

After this, the application process is the same and anyone interested in a secondment would need to complete the online application form.
