About the Opportunity

The Community Health Team (CHT) focuses on wellness group programming, wellness navigation and partnership with community organisations. CHT wellness programming includes risk factor management, healthy eating, physical activity, mental wellness and parenting. Reporting to the Health Services Manager, the Wellness Facilitator demonstrates flexibility and innovation in helping residents access services, programs and resources that support healthy behaviours. The role involves providing guidance, information and encouragement to meet health and wellness needs, and collaborating with primary‑care providers and community groups to maintain awareness of available supports. The facilitator works closely with the inter‑professional team to develop, implement and evaluate community‑based wellness programming. Collaboration, evaluation of team effectiveness, problem‑solving and clear communication skills are essential.

About You

We would love to hear from you if you have the following:

  • Bachelor’s degree in Nutrition (required)
  • Licensure by Nova Scotia Dietetic Association with completion of an accredited Dietetic Internship (required)
  • Minimum 1‑year relevant experience (clinical, primary health care, community‑based team settings)
  • Facilitation/education skills using adult education, self‑management supports and motivational enhancement (individual and group)
  • Experience working with people living with chronic condition(s)
  • Experience in partnership development, stakeholder collaboration and community development
  • Knowledge of population health, health promotion strategies and behaviour change principles
  • In‑depth knowledge of local health authority and community‑based services
  • Person‑centered/family‑focused, diversity‑relevant approaches to care
  • Strong interpersonal, teamwork, oral and written communication skills
  • Flexibility, adaptability and willingness to change
  • Innovation in program planning, implementation, evaluation and quality improvement
  • Computer skills (MS Office, email, internet)
  • Annual CPR‑AED certification and annual WHMIS training and certification
  • Valid Nova Scotia driver’s licence and reliable vehicle
  • Competencies in other languages preferred; French an asset

Hours

Long Assignment, Full‑time position, 75 hours bi‑weekly. Begins February 9, 2026 for a duration until February 26, 2027 (dates subject to change). Shifts may include evenings, weekends and holidays.

Compensation and Benefits

Hourly: $37.27 – $51.62. Eligible for a benefits package that includes health, dental, travel, long‑term disability, life insurance and a defined benefit pension plan.
