This is an exciting opportunity to join the Transforming Lives Team working alongside the Community Forensic Intellectual Developmental and Disability Service (CFIDD) as an experienced Social Worker.

Key Responsibilities

  • Undertake Care Act assessments, support planning, risk assessment, commissioning, and support discharges of forensic patients with learning disabilities and/or autism.
  • Provide ‘Social Supervision’ and report back to the Ministry of Justice for individuals subject to section 41 of the Mental Health Act.
  • Work creatively with adults who have care and support needs, promoting independence and wellbeing while managing risks.
  • Collaborate closely with the CFIDD multi‑disciplinary team and external agencies, maintaining a high level of confidence and independence.
  • Keep effective communication, negotiation, organisational and IT skills to develop flexible working practices.

Essential Skills and Experience

  • Good understanding and experience of the Care Act 2014, Mental Capacity Act 2005 and Mental Health Act 1983.
  • Experience of working with adults who have learning disabilities and/or autism in the community.
  • Registered with Social Work England and possess a valid driving licence with access to a vehicle for work purposes.
  • Demonstrated ability to undertake risk management in a small, complex caseload.
  • Strong communication, negotiation, organisational and IT skills.
