Urgent Community Response Clinical Assessor

The Urgent Community Response (UCR) Team is recruiting!
We are currently recruiting one new full‑time clinical assessor to be based at Parkhouse in Gedling, working across South Nottinghamshire (Gedling, Broxtowe and Rushcliffe).
UCR provides holistic, person‑centred care aimed at preventing unnecessary hospital admissions. The team works closely with EMAS, primary care, social care, community health partners, and other statutory and voluntary organisations.
All staff within the service collaborate with patients to enable independence, improve health outcomes and minimise admissions to hospital, long‑term care and social care packages.
Main duties of the job
UCR is an NHS England national service that reviews patients in their usual place of residence within 2 hours to reduce the risk of avoidable admission.
As part of the team you will be supported by colleagues, extend your clinical reasoning and practical skills, and be nurtured in your career.
The service operates 7 days a week from 08:00 to 22:00.

Job responsibilities
You will respond to patients in crisis, helping vulnerable clients when they need us the most. The role is ideal for nurses, physiotherapists, occupational therapists and other AHPs within a new, innovative UCR service.
Clinical assessors work across three bases: Cotgrave (Hub in Rushcliffe), Park House in Gedling and Stapleford Care Centre in Broxtowe.
Key responsibilities include:
- Analyse and develop treatment plans collaboratively within a multidisciplinary framework, taking a lead role as indicated.
- Undertake holistic assessments of health and social care needs, including comprehensive physical checks and use of stethoscope, percussion, and palpation techniques.
- Prioritise patients’ presenting conditions through clinical reasoning based on current professional opinion and the client’s prognosis.
- Manage a caseload of moderate to complex needs, supporting rehabilitation and reablement at patients’ homes to reduce future admissions.
- Demonstrate excellent communication and interpersonal skills, establishing effective working relationships with patients, staff, and key stakeholders inside and outside the Trust.
You must be a Registered Nurse (RGN or RN (Adult)) or Allied Health Professional with relevant clinical experience and evidence of continuing professional development.
A full UK driving licence and vehicle for business use are required. Reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Person Specification
Contractual
- A full UK driving licence and vehicle for business use are required; reasonable adjustments are available for disabled individuals.
Qualifications
- RGN/ AHP registered with an accountable professional body (NMC or HCPC).
- Degree level qualification or equivalent knowledge and experience in a health‑related subject.
- Evidence of continuous professional development.
- Management or leadership qualification.
Knowledge
- Competent in comprehensive physical assessment and use of NEWS2/SBAR.
- Ability to respond appropriately to complex scenarios using intuition, assertiveness and clinical reasoning.
- Competent with stethoscope use.
- Understanding of national UCR guidance.

Experience
- Substantial post registration in a relevant clinical setting, including acute and community.
- Experience of clinical case management and acute episode care.
- Experience within a UCR/rapid response/crisis team.
- At least 3 years post‑registration experience.
- Experience of clinical audit and prescribing equipment.
