Don\’t miss out on this fantastic opportunity to join our Nottingham West Community Therapy Service as an Occupational Therapist working in the community. If you want to be part of a forward-thinking and innovative service, then this is the opportunity you may have been looking for.

In this role, you will work closely with Physiotherapists, Assistant Practitioners, support staff, district nurses, specialist nurses, and social workers to provide the best possible care to service users in their own homes.

We are working towards a 7-day service – 8am to 8pm on a rota basis. Self-rostering is encouraged.

An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management, palliative care support, and support for timely discharges from our acute Trust partners.

You will work autonomously as well as part of the multidisciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness, and enable health, wellbeing, and self-care. You will be visiting patients within their own homes using a goal-oriented focus to maximise patient independence, safety, and quality of life.

The post will involve cross-site working and community visits.

A full UK driving licence and vehicle for business use are required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
