An exciting opportunity has arisen for a Senior Community Occupational Therapist to join a new service in the Virtual Ward team. Virtual wards are a national initiative and defined as \”A Virtual ward is a safe and efficient alternative to NHS bedded care which is enabled by technology\” NHS England and NHS Improvement. Notts Integrated care system aims to deliver this through a Nottinghamshire wide provider collaborative to deliver an ambition of 400 – 500 beds across Nottinghamshire by Dec 2023. Virtual wards allow patients to receive care, remote monitoring and treatment they need in their own home, safely and conveniently rather than in hospital. As a member of the Virtual Ward Multidisciplinary team the role of Community Occupational Therapist will support patient care to be delivered in the home providing an alternative approach to receiving hospital care or clinical monitoring. The Virtual Ward position while based at various sites delivers a joined-up approach across the Nottinghamshire Integrated Care System (ICS).

To offer a comprehensive nursing/therapy service to the resident population of Nottinghamshire, providing a multi-disciplinary and multi-agency approach to maximise patient function, wellbeing and self-care planning. To offer a comprehensive, needs-led Therapy service to patients with rehabilitation needs who are on the virtual wards pathway which will enable patients to be cared for in their own home thereby preventing unnecessary hospital admission.
