Community Support Worker

Overview

An exciting opportunity has arisen for a Community Support Worker to join our well established Intensive Community Assessment & Treatment Team based in Nottinghamshire. We are looking for someone with enthusiasm and energy who wants to make a difference to people\’s lives; is creative, innovative and highly motivated.

About ICATT

ICATT provides a rapid, short term assessment and treatment service for people with an Intellectual Disability who challenge services and may have additional mental health needs. The Team seeks to provide an alternative to hospital admission where it is appropriate to do so, enabling service users to receive treatment and care within their own homes, remain involved in their care as part of their own community and therefore minimising the disruption of their lives and the lives of those caring for them.

Role Overview

The post would suit someone who has a sound knowledge of and experience in working with people with an Intellectual Disability. You will be required to work with a delegated caseload and carry out relevant physical and mental health observations and interventions, to undertake nursing, therapy and social activities, support and monitoring of service users living in the community.

Responsibilities

• Participate fully as a multidisciplinary team member working in partnership with social care and voluntary/independent sector providers.
• Provide holistic advice and health education to service users, their carers and community support agencies.
• Exercise clinical judgement in respect of assessing and managing risk, and help deliver a safe, timely, caring and responsive service.
• Have assessment and care planning skills and experience in multi‑disciplinary working and supporting people with an Intellectual Disability and or Autism.
• Facilitate time limited therapeutic treatments and care for individuals on your delegated caseload.
• Record and report clinical observations and assist in treatment planning in the community, noting any physical and mental health changes in a timely manner.
• Liaise closely with other health and social care professionals as well as with patients\’ family and carers.
• Be resilient and prepared to focus on positives rather than the challenges.

Qualifications / Skills

• Sound knowledge and experience in working with people with an Intellectual Disability.
• Excellent communication skills.
• Clinical judgement in assessing and managing risk.
• Experience in nurturing and providing holistic care, including nursing, therapy and social activities.
• Experience in multi‑disciplinary working and supporting people with an Intellectual Disability or Autism.
