The Bassetlaw Older People Care Unit (OPCU) provides assessment, treatment, and support for older adults with mental health difficulties and individuals of any age with organic mental health diagnoses. The team works within the Bassetlaw locality alongside service users, carers, and partner agencies to promote recovery, independence, and quality of life. The County Dementia Outreach Service operates as a "virtual team" delivering person‑centred, dementia‑friendly care. The service collaborates with patients, families, and care home staff to reduce distress, manage behaviours of concern, prevent inappropriate hospital admissions, and maintain suitable care placements.

Main duties of the job

  • An experienced, NMC Registered Band 6 Community Psychiatric Nurse with substantial experience of working with people with dementia in an inpatient/residential setting and or community setting.
  • Advising & support care home staff to improve their skills and confidence to manage behaviours & promote person centred care planning.
  • Specialist knowledge of the nursing issues associated with dementia in a residential setting, high level of communication skills are crucial as are good written, verbal and IT skills.
  • Robust understanding of risk assessment and management, experience of managing a case load essential.
  • Demonstrate understanding of the mental and physical health and social needs of the client group, demonstrate a working understanding of Safeguarding issues and DOLs.
  • Time management and organisational skills and ability to work under pressure, nursing student mentor.

Job responsibilities

  • We are looking for a committed and experienced Band 6 Community Psychiatric Nurses to help deliver the service in Bassetlaw as part of Older People Care Unit.
  • You will be based within the Multi‑Disciplinary Community Mental Health Team whilst also being a part of the wider outreach service which is managed by a clinical specialist.
  • A key aspect of the Community Psychiatric Nurse role will be advising and supporting care home staff to improve their skills and confidence to manage behaviours and promote person centred care planning.
  • The County Dementia Outreach service covers the County of Nottinghamshire and operates within a virtual team service model with clinicians being based within locality based community mental health teams.
  • Applicants must be NMC registered.

Person Specification

  • Qualifications: RMN. Specialist knowledge of the nursing needs and issues associated with dementia in a residential setting. NMC. Non‑medical prescribing.
  • Training Experience: Substantial experience of working with people with dementia in an inpatient/residential setting and or community setting. Experience of managing a case load. Care planning within the MCA and best interest decision making experience.
  • Knowledge: A robust understanding of risk assessment and management. The 6 C's. Demonstrate understanding of the mental and physical health and social needs of the client group. Demonstrate a working understanding of Safeguarding issues and Deprivation of Liberty Safeguards. National and Local Nursing issues.
  • Skills: Effective, adaptable communication with patients, carers and other agencies. Good written, verbal and IT skills. Effective time management and organisational skills. Ability to work as part of a multidisciplinary team/inter‑agency team. Ability to work under pressure. Can discuss issues involved in managing a shared caseload. Experience of working in a team and to discuss the benefits of teamwork. Experience of delivering Learning and development opportunities for others.

Contractual requirements

  • A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
  • Ability to work unsocial hours as per service need.
