Overview

The COPD team has an exciting opportunity within its service. We are looking for a dynamic nurse to compliment our already established team. We are looking for a kind, dedicated team member with a keen interest in COPD care.

The post holder will demonstrate a high level of specialist respiratory knowledge providing care, guidance and support to patients, carers and healthcare professionals managing COPD both on inpatient wards and in outpatient clinic settings.

Responsibilities

  • Assessing and care planning for patients with COPD
  • Medication review and optimisation
  • Providing support and referrals (both secondary and primary care)
  • Educating patients, families and caregivers and coordinating care
  • Supporting consultant colleagues in clinics
  • Participating in regular MDT meetings to optimise care for our patient group
  • Working independently and as part of the MDT after a supernumerary period
  • Engaging in continuous education opportunities once in post
  • Participating in audit and service development
  • Reviewing inpatients with COPD and contributing to their care plans in hospital
  • Providing specialist input when COPD is a factor in admission for reasons other than COPD itself
  • Reviewing patients with COPD in specialist outpatient settings and nurse clinics
  • Supporting consultant COPD specialist clinics
  • Referring patients to MDT members across primary and secondary care
  • Attending specialist MDTs within secondary care and between primary and secondary care
  • Actively taking part in clinical audits to improve patient care

Qualifications and Skills

  • Respiratory experience of 2 years is essential
  • Competent IT skills are required
  • Clear communication and documentation skills are essential

Additional Information

This role will require working across both City and Queen Medical Centre campuses. The COPD nurse specialists review inpatients admitted to NUH with an exacerbation of their COPD. You will assess, plan, implement and evaluate their COPD care based on principles that are research-based. You will be asked for specialist input with patients where their primary reason for admission may be something other than their COPD but where it may be impacting on their management. You will review patients with COPD in a specialist outpatient setting in a nurse clinic. You will support consultant COPD specialist clinics. You will refer patients to identified members of the MDT across primary and secondary care. You will attend specialist MDTs both within a secondary care setting and between primary and secondary care. You will actively take part in clinical audit to improve patient care.
