Job summary

Nottingham CityCare Partnerships Integrated Care Homes Service (ICHS) aims to improve the experience of care home residents and staff by delivering high-quality, integrated, multidisciplinary NHS care. The service comprises of dementia outreach clinicians, physical therapy, and general nurses.

As an OT you will undertake specialist assessments of care home residents with physical therapy needs as well as those living with dementia; developing and implementing treatment plans. And using clinical knowledge to enhance physical and social environments, supporting individuals to engage in activities of daily living.

Applicants must demonstrate strong Occupational Therapy skills and experience supporting people ideally within community settings, and experience of working with people living with dementia. Experience in caseload management essential.

You will collaborate with team members and specialist services, benefiting from regular therapy and dementia-focused meetings, alongside peer support and supervision to aid ongoing professional development.

The Dementia Outreach and Therapy teams play a key role in enhancing residents wellbeing through person-centred rehabilitation and interdisciplinary expertise. Main duties of the job

This is a 12 month fixed-term role working 22.5 hours per week.

The Occupational Therapist is part of the Dementia Outreach Team (DOT) within the Integrated Care Home Service. The team includes dementia specialist therapists, RMNs and Assistant Practitioners. DOT therapists work closely with physical therapy clinicians as an integrated therapy team, supported by a therapy lead. The role requires strong clinical knowledge and experience in dementia care. The OT is responsible for holistic patient assessment, developing individual therapy programmes, and managing a clinical caseload. The post holder will supervise Assistant Practitioners and act as a resource for colleagues. Collaborative working is central, both within the service and with therapists from other services supporting care homes. Regular therapy meetings led by the clinical lead promote shared learning and innovative practice. The service supports care home staff to deliver high-quality care through training and education sessions, helping to build skills and confidence. It also promotes joint handovers within care homes to support proactive care and reduce avoidable hospital admissions.

Job responsibilities

Job Purpose

• To undertake specialist assessments on how to maximise the physical and social environments to help people engage in activities of daily living and contribute to multi-disciplinary treatment planning
• To be responsible for continuing the ongoing development of high standards of clinical practice and expertise within the service contributing to implementation of specific changes to practice and service protocols and making a professional contribution to the work of a multi-disciplinary team
• To advise and support care home staff in maintaining a residents wellbeing and engagement despite their skills declining.
• To manage/care coordinate a caseload of patients and will provide advice, education and teaching sessions to providers of residential/nursing care for people with dementia.

Dimensions

• To be part of a multi-disciplinary Integrated Care Homes Service, responsible for the delivery of an occupational therapy service to residents with mental health problems related to old age, particularly those with dementia.
• To supervise less experienced staff and students working with the Post Holder. To supervise, educate and assess the performance of undergraduate Occupational Therapy students as required ensuring the standard of practice and teaching meets the standards set by the degree level qualification.
• To increase the capacity, confidence, competence, and willingness of partner organisations to provide the right level of care to this vulnerable client group and by doing so increase standards of care, improve quality of life, increase independence and choice
