Nottingham CityCare Partnership is seeking an experienced Advanced Clinical Practitioner to join our Virtual Ward and Reablement Team. The ACP will deliver rapid clinical assessment in the community, prevent avoidable hospital admissions, and support safe discharge.
You will lead clinical decision-making in a multidisciplinary team, provide advanced assessment, prescribe where appropriate, and contribute to service development and staff training.
