The Clinical Beds team provides holistic assessments to patients who require a period of rehabilitation and assessment prior to going home after being discharged from hospital. We work closely as a multidisciplinary team alongside our partner services such as Adult social care and continuing health care to aid patients and families make long term decisions about their health and social care needs. This is an excellent opportunity to broaden your skill set in a holistic manner working with physiotherapists, occupational therapists and mental health nurses along with supporting junior colleagues such as student nurses, assistant practitioners and rehabilitation support workers.
Main duties of the job
- Work as an autonomous practitioner, undertaking comprehensive assessments to identify and meet the nursing needs for individuals under the care of the clinical beds team.
- Responsible for carrying out profession specific and holistic assessments, creating and evaluating care plans as required.
- Plan and develop goal focused rehabilitation/reablement programmes for adults with a wide variety of complex physical and mental health needs.
- Be an experienced registered nurse with knowledge and experience of community services.
- Prioritise workload, have excellent communication skills and build therapeutic relations with partner services.
Job responsibilities
- To work as an autonomous practitioner, undertaking comprehensive assessments to identify and meet the mental health needs for individuals under the care of community beds.
- To carry out profession specific, and holistic, assessments, care plans and evaluation as required.
- Assist the multi-disciplinary team to plan and develop a goal focused rehabilitation/reablement programme.
- Responsible for continuing the ongoing development of high standards of clinical practice.
- Work across different bases and community settings.
- Supervise, educate and assess the competency of junior staff, support staff and students.
Key Responsibilities
- Support the Team Manager in implementing and evaluating targets and objectives.
- Advise the Team Manager in relation to clinical needs of the client group.
- Ensure the health & safety of self and others at all times.
- Use information technology on a daily basis and maintain clear, concise and timely records.
- Participate in the Lone Worker Tracking procedure.
- Act as duty clinician for the process of referrals.

- Be professionally and legally responsible and accountable for all aspects of your work.
- Gain valid informed consent and identify clients who lack capacity.
- Undertake advanced comprehensive assessment and evaluation of adults.
- Provide highly specialist advice, education and mentorship to other staff.
- Drive quality, clinical service improvement and clinical excellence.

- Maintain a high standard of care to clients and carers.
- Facilitate and carry out 1:1 and group education and training.

Person Specification
- Registered Nurse with NMC register.
- Post registration mental health experience.
- Evidence of further Professional Development.
- Ability to be flexible over hours worked within contracted hours.
- Minimum of 2 years post graduate experience at Band 5.
- Experience of working in a multi-disciplinary team.
