The predicted salary is between 37338 - 44962 Β£ per year.
As a member of the multi-disciplinary Dementia Outreach Team within Nottingham CityCare Partnerships Integrated Care Homes Service (ICHS), you will undertake specialist assessments of care home residents living with dementia. You will develop and implement treatment plans, using clinical knowledge to enhance physical and social environments, supporting individuals to engage in activities of daily living.
This is a 12 month fixed-term role working 22.5 hours per week.
Responsibilities- Perform holistic patient assessments and develop individual therapy programs.
- Manage a clinical caseload of care home residents with dementia.
- Collaborate with team members, specialist services, and physical therapy clinicians within the integrated therapy team.
- Supervise Assistant Practitioners and act as a resource for colleagues.
- Deliver training and education sessions to care home staff to support high-quality care.
- Promote joint handovers within care homes to support proactive care and reduce avoidable hospital admissions.
- Strong Occupational Therapy skills and experience supporting people with dementia, ideally within community settings.
- Experience in contracture management.
- Effective caseload management experience.
