Nottingham CityCare Partnership is seeking an experienced Advanced Clinical Practitioner to join the Virtual Ward and Reablement Team in the community. The ACP will lead autonomous clinical assessment, diagnosis and treatment for adults in acute health or functional crisis, prevent hospital admissions and support timely discharge.
You will work within a multidisciplinary team, provide mentorship, contribute to service development and staff training, and uphold standards of care across CityCare
