Nottingham CityCare Partnership CIC is seeking an Assistant Practitioner to join their Community Supportive and Palliative Care Team. This role is based at Radford Health Centre providing specialist support to palliative patients and their families across Nottingham City.
The Assistant Practitioner will work flexibly over 30 hours a week and support the delivery of high-quality palliative end-of-life care, enhancing patient and carer experiences.
