Join our Community Supportive and Palliative Care Team as an Assistant Practitioner (Band 4, 30 hours / 0.8 WTE). The role is based at Radford Health Centre and provides specialist support to palliative patients and their families across Nottingham City, including in patients' own homes. The Assistant Practitioner works flexibly over 4 days Monday–Friday 8:30‑16:30 (excluding bank holidays) and supports the Nursing and Rehabilitation staff. The role improves the delivery of palliative end‑of‑life care in the community for patients with a life‑limiting diagnosis, promoting independence and improving patient and carer experiences.
Main Duties
- Support patients with palliative and end‑of‑life care across Nottingham City, including homes and care settings.
- Work within a multidisciplinary team, manage own workload, maintain accurate records, and support people with palliative or long‑term conditions.
- Use strong communication and interpersonal skills, assess needs, support independence, contribute to care planning, and work closely with nurses, therapists and other professionals.
- Work flexibly, carrying out delegated clinical and rehabilitation tasks and supporting safe, high‑quality care delivery.
- Drive the team to deliver compassionate communication and effective liaison with other care providers.
- Maintain accurate patient records and documentation per organisational policy.
- Provide administrative, clerical and housekeeping duties to promote good practice within the team.
- Assist with triage, signpost to other services and support the triage nurse as required.
- Undergo and undertake any other duties reasonably required by clinicians.
Qualifications and Experience
- Foundation degree, NVQ Level 3, or equivalent, with demonstrable relevant experience in health, social care or voluntary care.
- Experience working within a multidisciplinary team, managing workload and maintaining accurate records.
- Experience supporting individuals or groups in health, social care or voluntary care settings with palliative or long‑term conditions.

- Driving licence and access to a vehicle for work purposes.
