Integrated Respiratory and Home Oxygen Service Community Support Worker

An excitingopportunity has arisen for a Band 3 Respiratory Community Support Worker tojoin Nottingham CityCares Integrated Respiratory Service working 30 hours perweek.

The IntegratedRespiratory and Home Oxygen Service are a proactive, forward thinking anddynamic team which provides evidence-based assessment and follow up forindividuals requiring urgent and routine respiratory management in thecommunity. The team provide an acute response service alongside chronicrespiratory condition management across health and social care sectors. Theteam are based at Aspect house but work across a variety of settings inNottingham city.

We are looking for an enthusiastic, motivated and conscientious teammember who is willing to learn and work within the established IntegratedRespiratory Service. You will undertakerespiratory routine clinical support in an integrated manner within the servicepathway. Within the wider service you will be involved in development andsupport to other health care professionals.

The hours of work will between the hours of 8am and 5pm Monday to Friday.

Main duties of the job

You will need to demonstrate excellent interpersonal and communication skills. You must be able to prioritize tasks and manage time effectively, also be able to work in an integrated manner and flexibly within the team and be confident in completing individual tasks.

You will need to hold a valid driving license and have access to a vehicle for this role.

Job responsibilities

Job Purpose

  • To work as part of the IntegratedRespiratory Service, providing integrated health and social care support topatients and carers in both clinic and exercise group as well as in the homeenvironment.
  • To undertake technical, clinical and a varietyof other appropriate activities following training and demonstration ofcompetency, delegated by the relevant profession
  • To work without direct supervision undertakingdelegated tasks in order to support the team
  • Toreport back to the team anything that may effect or contribute to the futurecare of the patients or carers

Key Responsibilities

  • Assist in delivery of Pulmonary Rehabilitationsessions.
  • Addressing Health and social need.
  • Implement programs of care assessedby Health and Social Care Professionals to address peoples health and socialcare needs in a variety of settings
  • Contribute to the reduction ofhealth inequalities through the delivery of agreed health promotioninterventions e.g. Smoking cessation
  • Undertake regular telephone dutiesand be able to advise patients appropriately and elevate to a trained memberof staff any concerns.

Person Specification

Experience

  • Ability to undertake non-routine activity in responding autonomously to individual patients needs within sphere of own competence
  • Ability to work alone without direct supervision
  • Experience of working in multidisciplinary health and/or social care team

Qualifications

  • Good standard of basic education
  • Basic IT Skills
  • NVQ Level 3 in Health & Social Care
  • ECDL

Knowledge & Skills

  • Knowledge of patient care in health and/or social care setting
  • Awareness of need for confidentiality
  • Effective written and verbal communication skills
  • Ability to undertake practical procedures
  • Able to work both autonomously and as part of multidisciplinary team
  • Uses person centred approach

  • Ability to plan ongoing clinical activity e.g. home visits, organising appointments/assessments
  • Provide education to clients and carers under guidance of qualified member of team

Special Requirements

  • Ability to be flexible over hours worked within contracted hours to meet the needs of the service
  • Ability to work out of hours
  • Full driving licence and the ability to travel between locations.
  • To participate in and fulfil the requirements of the Directors on-call arrangements.
