Community Public Health Nurse (Transient Families Team)
Nottingham CityCares Community Public Health 0-19 Nursing Service is excited to be expanding their Transient Families Team based at St Anns Valley Centre.
Nottingham City has a high transient family population with many refugees, asylum seekers and families in temporary accommodation. We have a dedicated team who deliver the Healthy Child Programme and support the needs of our asylum seekers, families in refuge and Irish travellers across Nottingham City.
We are seeking a Community Public Health Nurse for 2-year fixed term.

We support staff with their continued professional development and in partnership with Derby University host Specialist Community Public Health Nurse students for both School Nursing and Health Visiting. Many of our CPHN staff have progressed to Specialist roles.
The hours of work will be minimum 37.5 hours, working days will be discussed at interview.
The Service hours are Monday-Friday 8:00am 5pm excluding weekends and bank holidays.
Main duties of the job
You will be part of our Childrens Public Health 0-19 Nursing Service, working from a range of community settings and within clients homes. You will support the Specialist Community Public Health Nurse (SCPHN) (Health Visitor/School Nurse) in the delivery of targeted services.
You will support the SCPHN in the management of a defined caseload as part of a 0-19 years integrated multi-agency partnership approach to promote health and contribute to tackling health inequalities in line with local and national guidelines, public health priorities and safeguarding responsibilities.

Job Purpose
- The aim of The Childrens Public Health 0-19 Nursing Service is to reduce health inequalities and improve the health and wellbeing of children and their families in Nottingham City. The Transient Families Team work primarily with families seeking asylum and those in domestic abuse refuge. The team offers a targeted service for all children and young people in Nottingham City as set out in the Healthy Child Programme.
- The role of the Community Public Health Nurse is:
- To work within a multi-disciplinary team, supporting the Specialist Community Public Health Nurse School Nurse (SN)/Health Visitor (HV) in the delivery of the universal and targeted services in a variety of community settings including citywide working.
- To support the SN/HV in the management of a defined caseload, as part of an 0-19 years integrated multi-agency partnership approach. Working to promote health and contribute to tackling health inequalities in line with local and national guidelines, public health priorities and Safeguarding responsibilities.
- To work as part of a multi-disciplinary team sometimes without direct supervision but reporting back to the SN/HV on a regular basis.
- To work with our most vulnerable children within Nottingham City with the expectation to attend safeguarding meetings to include ICPC, RCPC, Core Groups, LACS, ERMM, Strategy Meetings, liaising with the caseload holder.
- To use a child & family approach to working, in a solution focused way.
- To promote holistic healthcare to the babies, children, young people, and families and to work in partnership with other agencies.
- To lead on specific programmes of care for babies, children and young people who have specific/complex needs.
- To lead on and participate in health promotion initiatives e.g., childhood immunisations, newborn blood spots, in line with local and national guidelines, public health priorities and safeguarding responsibilities.
- To support the development of the care pathways required for this population group to reach their optimum potential.
- To provide advice, signposting and support for parents, carers and young people using Text Health, our text messaging service.
- To ensure that the rights, needs and dignities of babies, children, young people, and families are met by providing health promotion and appropriate programmes of care.
- To promote the health of babies, children, young people to enable each child to reach his/her full development and educational potential which supports the philosophy of inclusive education.
- To work in an inner city with a rich and varied ethnic, cultural, and socioeconomic climate.

Key Responsibilities
Clinical
- To follow a competency-based induction and training pathway supported by your mentor.
- To support the Children and the Community Public Health Nursing Team to deliver in Healthy Child Programme 0-19 years. This will include development reviews, the National Childhood Measurement Programme.
- To undertake home visiting programmes to offer support to carers/parents in the care of their children following competency-based training supervision.
- To work in a variety of settings including clients homes, Childrens Centres, health premises and education settings across Nottingham City.
- Following allocation of appropriate work relating to babies, children, young people, and families by the SN/HV, the post holder will have responsibility for the assessment; development; implementation and evaluation of programmes of care for individual children and those children with complex health care needs and their families
- Ability to organise and prioritise own workload under the supervision of senior members of the team.
- You will be able to use appropriate evidenced based tools to assess the physical and emotional wellbeing of babies, children, young people, and families

- To prepare reports, under supervision, for child protection, children in need, Looked after Children, Common Assessment Framework and Priority Families. Take the lead professional role and attend meetings and reviews where appropriate.
- To conduct oneself in a corporate and professional manner always maintaining confidentiality including compliance with the CityCare dress code policy.
- To attend essential, mandatory, and appropriate training courses and meetings, thus keeping professional development current. Ensure practice is evidence based to maintain high quality, effective public health care.
- To demonstrate leadership and contribute to the appraisal, development, and monitoring of performance of team members induct and mentor staff and students.
- To undertake mentorship, preceptorship, and clinical supervision as per policy.
