Nottingham City General Practice Alliance is seeking a Social Prescribing Link Worker on a fixed-term basis to join City South PCN 7. The role supports independence, wellbeing and resilience by signposting to community services within a multidisciplinary team.

You will manage your own caseload, provide information and support to patients, and act as the link between the patient, GP and other service providers to improve health outcomes.
