Job overview: Due to expansion and career progression within our team, we have an exciting opportunity for a Clinical Pharmacist to join our organisation. We are looking for an enthusiastic and dynamic pharmacist to work within Nottingham City South PCN. Completion of CPPE pathway and independent prescribing are desirable but not essential. 2 years experience of pharmacy work within the healthcare setting or equivalent is desirable but not essential.
Main duties of the job: The post holder will work as part of a multi-disciplinary team in a partially patient facing role. The post holder will be responsible for areas of chronic disease management, clinical medication reviews, supporting the general practice staff with regards to prescription and medication queries. They will potentially provide support for the repeat prescription system, deal with accurate prescription requests, medicine reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicine advice, while providing clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

Person specification: Interview achieved grade / years experience worked within a primary setting previously completion of CPPE pathway and independent prescribing.
