Job overview: The post holder will work within the PCN Team, providing coordination, administrative and support to the clinical teams and other members of the PCN.
Main duties of the job:
- The post holder will be primarily responsible for coordinating the PCN response to the Directed Enhanced Service requirements across PCN GP practices.
- There will be a strong focus on collation of practice data, identifying and understanding patient cohorts, patient engagement, and administrative tasks that support the smooth and effective running of the PCN.
- Administrative support will include activities such as organising meetings, recording minutes, and action logging.
- Excellent telephone communication skills are required to support patients and external providers such as care homes and safeguarding teams.
- Excellent IT skills are necessary, along with the ability to work in a fast-paced environment and under pressure.
- Enthusiasm and self-motivation are essential.
- Teamwork is important, but the ability to use initiative is also required.
- Experience with SystmOne and EMIS is ideal, along with a good understanding of healthcare; however, full training and development will be provided.

Detailed job description and main responsibilities:
- Support the PCN by providing administrative support to the DBTM Manager, including meeting support, agenda setting, recording, producing, and reviewing structured minutes and recorded actions.
- Work autonomously, reporting to the DBTM Manager to support project work across the PCN, including gathering and presenting data sets in line with current and future PCN projects.
- Manage electronic, paper, and telephony PCN correspondence.
- Provide PCN workflow management, reviewing systems to track deadlines and working with colleagues to diarise and support PCN meetings, workflow, and deadlines.
Person specification:
- Good standard of written English (at least GCSE at a higher grade or equivalent).
- Numerate and able to work with data (at least GCSE maths at a higher grade or equivalent).
- Experience providing advice/signposting to patients or clients.
- Experience of coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes.
- Experience of using technology and digital tools to support health and wellbeing.
- Direct experience of working within primary care (GP surgeries).
