About the Role
A vacancy has arisen within the Pathway Team for a Reviewing and Enablement Officer. The Pathway Team works directly with citizens who have a learning disability, providing face-to-face support to help them develop practical life skills that promote confidence, independence, and wellbeing. This includes support with cooking, cleaning and maintaining a safe home environment, money management, travel training, accessing education and college opportunities, and engaging in voluntary work. The team also helps citizens to understand and manage their safety both at home and within their local communities.
The role of the Reviewing and Enablement Officer is to work closely with citizens to help them achieve their full potential. Officers will research and provide information about a range of community-based resources and opportunities that support the outcomes identified by the citizen and are available within their local area. The support and information provided will be tailored to each individual and focused on meeting their eligible social care outcomes, as identified through a strengths-based assessment and action plan. The aim of this work is to enable citizens to maximise their independence, reduce reliance on paid support where appropriate, and make progress towards achieving their desired outcomes.
This is a job-share post, following the existing post holder reducing their working hours to part-time.
Key Responsibilities include:
- To develop and maintain specialist knowledge of community-based opportunities at a neighbourhood level, and ensure these opportunities are shared with ‘local offer’ outlets.
- To use a strength-based approach to promote independence, set outcomes, enhance well-being through connecting people, and communities.
- To support the WLD teams by providing clear outcomes at the end point of their work to promote continued progression and review.
- To work with communities and community groups to create social hubs and opportunities, and identify opportunities to develop such hubs.
- To promote the principles of co-production and supported volunteering to community organisations.
- To actively encourage, support and develop Citizen’s skills, preferences and experiences and support Citizen’s to identify their outcomes and aspirations.
About You
The ideal candidate will have:
- Experience of working with people with disabilities.
- Ability to identify and assess risk, write, implement and review risk assessments in relation to the provision of community-based services and vulnerable adults.
- Ability to communicate clearly, orally and in writing, and maintain accurate and up to date information and records, where necessary inputting information onto a relevant IT/electronic system.
- Awareness of 2005 Mental Capacity Act, Deprivation of Liberty and Best Interest Decision making.
- Strong interpersonal skills to gain the agreement and acceptance of others including colleagues, senior managers and customers.
- Ability to make decisions and solve problems to meet operational targets, involving devising solutions and prioritising the resources available.
A DBS enhanced check for a regulated activity is required for this post.
