Nottingham City Council is seeking a Children s Continuing Care Coordinator (Grade G) to oversee timely, effective and coordinated support for children and young people with complex health needs through the Continuing Care process.

The role involves safeguarding, and working across social care, health, education, finance and the Integrated Care Board to ensure robust funding arrangements and timely care planning.

Hybrid working with a full-time (37 hours) contract and a permanent post.
