About the Role:
The Nottingham Health and Care Point (NHCP) is the front door for adult social care referrals in Nottingham City. We are looking for 5 motivated and compassionate Health and Social Care Officers (HSCO's) to join our busy and supportive team, helping residents access the right care, support and information at the right time. As an HSCO, you will be the first point of contact for individuals, families, carers and professionals seeking advice and support in relation to adult social care needs. You will undertake strengths-based conversations, gather and assess information, identify risks and needs, and provide timely advice, guidance and signposting to a wide range of services. Your work will play a vital role in promoting independence, preventing escalation of need and ensuring people receive appropriate support as quickly as possible.
About You:
- Experience of working within a health, social care, customer service or an advice-based environment, supporting people with a range of needs and delivering high-quality, person-centred services.
- Experience of handling sensitive information, managing competing priorities and working effectively within a multi-disciplinary setting.
- Knowledge of adult social care services, safeguarding principles, strength-based practice and the importance of promoting independence and wellbeing.
- An understanding of the challenges faced by vulnerable adults, carers and those with complex needs.
- Excellent communication and interpersonal skills, enabling you to build rapport with citizens, carers, professionals and partner agencies.
- Strong listening, problem-solving and decision-making skills along with the ability to gather and analyse information, assess needs and identify appropriate support options.
- Confidence in using IT systems, maintaining accurate records and an ability to manage a varied workload in a fast-paced environment.
- A commitment to delivering compassionate, inclusive and responsive services that put citizens at the heart of everything you do.
A DBS enhanced check for a regulated activity is required for this post.
