About the Role:

  • Care Workers at Level 2 implement Care Plans and report any concerns or reasons care plans cannot be followed.
  • Monitor care and welfare of citizens, recording accurate information on running records, communication books, care plans monitoring charts (e.g. weight monitoring, falls, etc.) and other written and electronic communications as required.
  • Work and cooperate with other professionals, as well as citizens and their meaningful others, to ensure a shared approach to care and support and continuity of care and treatment.
  • Provide safe and appropriate personalised care and support to citizens in a way that maintains their dignity, maximises independence, and gives them choice and control over the way their care is provided.
  • Write and maintain individual person-centred care plans and administer medication.
  • Work in any area of Adult Social Care, whether in the home, the community, or within a Council establishment, and be prepared to work with citizens with different and varying needs.

About You:

  • Ability to assist and support citizens to exercise choice and control in accordance with their needs, preferences, and aspirations.
  • Encourage, motivate, and support citizens to maintain or regain maximum independence in all aspects of daily living and purposeful activities.
  • Develop and coordinate leisure and social activities that promote self-esteem, confidence, and a purposeful day.
  • Ability to write and maintain up-to-date individual person-centred care plans for citizens.

A DBS enhanced check for a regulated activity is required for this post. Please note that this role does not meet the relevant salary threshold criteria for Skilled Visa sponsorship, and therefore applicants requiring sponsorship are unable to apply.
