A fantastic and exciting opportunity has arisen within Norton Canes Practice for a committed, enthusiastic and self-motivated individual to join the team. As a General Practice Advanced Nurse Practitioner, you will work as part of a clinical team, with GPs, Mental Health Practitioners, Practice Nurses, Pharmacists, Physiotherapists and Healthcare Assistants across the practice and within our PCN. The post holder will provide high-quality, patient-centred care to the patients of Norton Canes Practice by diagnosing and prescribing treatments and medication while working within defined procedures and protocols as laid down by the GPs.
Main duties of the job
- To provide high-quality, patient-centred care as part of a team, contributing to the delivery of safe, efficient, and proactive healthcare services. This includes diagnosing and managing acute and chronic conditions, and supporting health promotion and prevention initiatives.
- Working as part of the on the day acute care team managing presentations of acute illness with support of the duty GP.
- Deliver safe, effective, and evidence-based care and treatment for a wide range of conditions within your knowledge.
- Actively engage with patients to promote healthy lifestyles, and facilitate referrals to community and specialist services.
- Work closely with GPs, nurses, and other healthcare professionals to deliver integrated care within defined healthcare pathways.
- Support the Practices administrative workload, and contribute to clinical audits, risk assessments, and service evaluations.
About us
Our Practice consists of 1 GP Partner, 1 Practice Nurse, 1 Healthcare Assistant, 4 Receptionists and 1 Practice Manager. We also have the benefit of a Social Prescriber, a First Contact Physiotherapist, Dietician, Mental Health Practitioner Team and Pharmacy Team. You would be entitled to join the NHS Pension Scheme.
Person Specification
- Clear Communicator with Clinical and Non-clinical staff.
- Working within own capabilities.
- Working autonomously in an advanced nursing role.
- Minimum of 1 years experience in General Practice.
- Able to use EMIS.
- Able to use DOCMAN.
- Experience of regular auditing.
Qualifications
- 1 years experience in General Practice.
- Long term conditions qualification.
- Experience in Triage.
- Registered Nurse with the NMC.
- L7 Minor illness and/or advanced history taking module.
- Commitment to completing advanced qualifications.
