Description

Northwestern Counseling & Support Services is seeking compassionate and dedicated individuals to join our Community Rehabilitation & Treatment (CRT) team. These case management positions serve adults diagnosed with severe mental illness. We are looking for individuals who are person-centered and passionate about working in the mental health field. These roles support individuals with high acuity needs who intersect with criminal justice, legal, and substance use systems, helping them lead more independent and fulfilling lives.

Key Responsibilities:

  • Providing services across community-based settings, including supportive counseling, symptom management, daily living activities, social and interpersonal skills development, money management, and advocacy.
  • Building rapport with individuals who may be difficult to engage or resistant to treatment.
  • Maintaining a positive attitude, working well within a team, and staying organized to manage caseloads effectively.
  • Collaborating and coordinating care with community resources, agencies, and internal or external service providers.
  • Developing and overseeing treatment plans, service coordination, crisis intervention, and representative payee services.

Qualifications:

  • Reliable transportation capable of transporting clients.
  • Experience in mental health case management is a plus, but training will be provided for the right candidate.
