Overview

Social Work Support Assistant – Home Safe Hospital Discharge Service at Northumbria Specialist Emergency Care Hospital, Cramlington.

The role is permanent, full time (37 hours per week) and will mainly work at Northumbria Specialist Emergency Care Hospital, with flexible cover required at other hospitals in the County.

Responsibilities

  • Work alongside individuals, families and carers to comprehensively assess needs, set objectives, develop care plans and deliver interventions to facilitate safe hospital discharges.
  • Complete Urgent Needs Assessments and Mental Capacity Assessments.
  • Collaborate with external services to access resources required to meet clients’ care and support needs.
  • Manage day‑to‑day caseload while receiving regular supervision and support from the Team Manager and social work colleagues.
  • Provide a good balance of autonomy and support in delivering high‑quality assessment and care services.

Salary range: £28,598 – £31,022 per annum.

Full‑time, 37 hours per week, permanent position.
