Are you an experiened and dynamic practitioner?

Northumberland County Council are looking for a motivated and passionate practitioner to take on an opportunity within their 14+ Social Care Team.

The successful candidate will work with young people who are leaving care and moving on to independent / semi-independent living.

We are looking for a practitioner who has direct experience of working within a multi-agency setting to work to address young people’s support needs as they transition to adulthood.

You will join an experienced and committed team of social workers and leaving care support workers who all share the same ethos around achieving the best possible outcomes for young people.

You will carry a manageable caseload of young people and also cowork cases alongside social workers to support a smooth transition to leaving care.
