Following recent service developments, we are delighted to offer exciting Social Work opportunities within our developing countywide Learning Disability Team. Post is permanent, full‑time at 37hrs per week. You will be based at Eddie Ferguson House, Blyth but we offer flexible working options, including hybrid work from alternative locality bases around the County. Responsibilities
- Undertake care and support needs assessments, care planning and reviews.
- Complete Care Act and Mental Capacity assessments.
- Work closely with individuals, families and carers to create tailored care plans.
- Collaborate with a range of internal and external statutory, private, charitable and voluntary stakeholders.
- Manage caseload autonomously, receiving guidance from Advanced Social Work Practitioners and Team Manager.
Qualifications
- Degree in Social Work and registered with Social Work England.
- Successfully completed the UK Skills for Care ASYE programme.
- Experience of Social Work with adults, particularly with clients whose primary need is a learning disability.
- Valid UK driving licence and must own a car for work purposes.
