Northumberland County Council is seeking a Social Worker to join the Home Safe Hospital Discharge Service at Wansbeck General Hospital in Ashington. The role is based primarily at Wansbeck General Hospital with flexibility to cover other county hospitals.
It is a 37-hour-per-week permanent position. You will assess psychosocial needs, develop care plans and coordinate appropriate interventions to ensure safe discharges, with supervision from a Team Manager and a solid understanding of the Care
