Transition Advisor: SEND
Role Responsibilities
- Work in partnership with schools to plan and deliver early transition support
- Assess student needs and develop tailored transition plans
- Manage a caseload of SEN students and track progress
- Contribute to EHCP processes, meetings, and reviews
- Liaise with families, staff, and external agencies
- Maintain accurate records and update relevant databases
Qualifications
- Level 2 or 3 qualification in learning support or equivalent
- Level 2 literacy and numeracy
Essential Skills & Experience
- Strong knowledge of SEN and EHCP processes
- Experience working with schools and external agencies
- Ability to assess needs and plan effective support
- Clear communication and relationship building skills
- Strong organisational and record‑keeping skills
- Ability to work independently and as part of a team
Desirable Skills & Experience
- Experience supporting transition into further education
- Familiarity with college systems for student tracking
- Experience contributing to EHCP reviews and planning
