About the Role
This role focuses on early, collaborative transition planning for students with SEN, working closely with schools to assess needs, design support, and prepare students for college life. You will play a key part in the EHCP process and ensure each learner has a clear, supported pathway. This is a strong opportunity to develop expertise in SEN support, multi‑agency working, and data systems, while contributing to meaningful outcomes for young people.
Role Responsibilities
- Work in partnership with schools to plan and deliver early transition support
- Assess student needs and develop tailored transition plans
- Manage a caseload of SEN students and track progress
- Contribute to EHCP processes, meetings, and reviews
- Liaise with families, staff, and external agencies
- Maintain accurate records and update relevant databases

- Level 2 or 3 qualification in learning support or equivalent
