Northgate Medical Practice is seeking two Care Navigators to join our reception team. You will be the first point of contact for patients, directing them on the phone and in person to the right support, working closely with clinicians and the wider team.
The role requires a full driving licence and the ability to travel between our three sites. It is a full-time position (37.5 hours) with shifts within opening hours and Saturday rota participation.
