Overview
An opportunity has arisen for a band 5 Nurse to join the forward thinking Intermediate care team. The caseload is predominantly focused on short term intervention during a period of rehabilitation and recuperation for patients following discharge from hospital or other settings. The role covers Sir John Mason house working closely with social care and GP\’s to support patients in returning home but also includes intervention across a range of settings and locations throughout North Lincolnshire. It will be expected that you will have an understanding and commitment to the continuing development and modernization of community nursing and must have effective communication and negotiation skills.
Responsibilities
- Undertake comprehensive and holistic assessment of the patient’s nursing care needs for complex patients within Intermediate Care
- Use this assessment to develop a plan of care, which is evidence based and planned in partnership with the patient whenever possible.
- Implement the planned programme of care, delegating duties appropriately and teaching and coordinating other members of the healthcare team, patient, family and carers.
- Evaluate the effectiveness of the nursing care provided and make adjustments to the care plan as necessary. Feedback to caseload holder changes in the patient’s condition as soon as is practically possible.
- Advise on the promotion of health and the prevention of illness as an integral part of nursing duties.
- Maintain clinical records in accordance with NMC “Guidelines on Record Keeping” and Trust policies.
