The Assistant Practitioner Therapy role will be within the Hospital team. The role involves offering assessments and treatments to optimize patients\’ mobility and discharge planning from hospital. This position is for a dual-trained assistant practitioner working closely with physiotherapists, occupational therapists, and other assistant practitioners to meet the expected targets.

You will work alongside qualified staff to provide high-quality therapy assessments and treatments tailored to the service user\’s needs. You will play a vital role in their discharge planning, assessing ongoing help and support needs or facilitating discharge home. Weekend working will be required.

There is a permanent and a 12-month fixed-term contract available.

Work within a competency-based framework to manage an allocated caseload of clients. Gather baseline information from various sources to enable comprehensive client assessment. Work independently or jointly with qualified therapists (dependent on competencies gained and level of complexity) to fully assess client needs, including physical, functional, social, and cognitive aspects. Plan, review, and modify treatment plans in line with client goals and outcomes achieved per intervention.

Take responsibility for managing your own caseload within required timescales, proactively seeking supervision and support. Ensure each contact is meaningful and seek supervision for tasks beyond current competencies. Monitor and adjust treatment formats according to clinical needs and client goals, using validated outcome measures with each individual.

Document interventions according to Trust standards on System One. Collaborate closely with other MDT members to provide holistic, person-centered care.

Audit responses to treatment, and develop or modify resources and working methods based on current research.

Engage actively in training, supervision, reflection, and annual reviews. Be proactive in seeking support when needed, managing risks as they occur, and ensuring appropriate activity levels as per individual Job Plans.
