We have an exhilarating opportunity for you to be a key player in our innovative, supportive Community Diabetes Service, dedicated to providing exceptional clinical care to adults in Bury, Heywood, Middleton, and Rochdale.
Overview
As a vital link between primary and secondary care, our mission is to prevent unnecessary hospital admissions and enhance life quality for patients. You will be at the forefront of transforming diabetes management by providing essential support, expert advice, and comprehensive education not only to patients but also to their families, carers, and health professionals.
Location & Schedule
Based at the modern Moorgate Primary Care Centre in Bury and the Phoenix Centre in Heywood, our community‑focused team delivers tailored diabetes interventions in various settings, ensuring that patients receive the personalised care they deserve. Hours of work are between 7.5 and 22.5 per week (please state preference on the application form).
Contract Details
9‑month secondment contract covering maternity leave. Full and part‑time hours are available.
Responsibilities
- Provide comprehensive clinical care to adults with diabetes.
- Initiate and adjust injectable and oral diabetes medications.
- Deliver education and support to patients, families, carers, and health professionals.
- Collaborate with multidisciplinary teams to monitor patients and prevent hospital admissions.
- Contribute ideas for future service development.
Qualifications & Experience
- Current NMC registration.
- Up‑to‑date knowledge of the diabetes speciality.
- Experience managing complex diabetes conditions.
- Experience in initiating and adjusting insulin and oral therapies.
- Commitment to ongoing professional development and access to clinical supervision.
