The HMR Rapid Response service has an exciting vacancy for a community-based Band 7 Clinical Practitioner within the team based at Rochdale Infirmary, supporting the residents of Heywood, Middleton, and Rochdale (HMR).

We are a community-based, multi-disciplinary team of skilled professionals providing holistic care and support to enable people to remain in their own homes and avoid unnecessary hospital admissions.

We focus on individualised, person-centred care, putting the patient first at all times.

The full-time vacancy offers an opportunity to join a team comprising advanced practitioners, physiotherapists, occupational therapists, pharmacists, and community nurses. It is open to appropriately qualified nurses and allied health professionals.

Regular supervision, competency training, performance reviews, and Continued Personal Development are actively encouraged.

As a Rapid Response Practitioner, you will be part of an exciting community-based workforce providing specialised input to meet the needs of the HMR population. You will develop and lead relationships across the rapid response team, prevent unnecessary hospital admissions, enable individuals to remain at home longer, and prevent premature admissions into long-term care facilities.

You will participate in the Hospital@Home service, ensuring residents of HMR receive timely and appropriate acute care at home.

You will assist individuals and their carers in proactively managing and anticipating health needs, supporting self-care, developing plans, and working collaboratively with the wider multidisciplinary team, including partner organisations such as NWAS, GP/primary care, pharmacy services, and wider Integrated Neighbourhood Team members.

You will be involved in developing, delivering, and evaluating high-quality, patient-centred services aligned with Business Group and Trust objectives.
