Overview

Under the direction of the Director of Population Health, the Program Specialist will be responsible for supporting the efforts and responsibilities of the Population Health program across the continuum of care to improve health, enhance the care experience, improve health equity, and reduce health care fragmentation. This includes, but is not limited to, assisting in the development and implementation of population health management strategies and accreditation and other programs by supporting providers and clinical practice sites in improving patient health outcomes and practice efficiencies. The Program Specialist will implement and monitor data-driven population health strategies and programs (e.g., NCQA recognition programs), identify variances in practice efficiencies related to recognition programs, and assist the Director of Population Health in executing improvement plans that supporting the value of meeting and exceeding national recognition requirements. Collaboration with providers, clinical staff and operations leaders is critical to success of this position.

  • Educational Requirements: Associates Degree, or other Clinical training or experience (e.g., Medical Assistant, Patient Care Tech).
  • Minimum Experience: 3 years experience in healthcare setting, project management, quality improvement, or other related experience.

Job Specific And Unique Knowledge, Skills And Abilities

  • Maintains current awareness and understanding of population health management strategies, accreditation, and other programs designed to improve health, enhance the care experience, reduce health care disparities, and improve practice site efficiencies.
  • Understands and is able to translate and communicate the requirements for achieving and maintaining recognition from the National Committee for Quality Assurance (NCQA) in multiple recognition programs, including, but not limited to, Patient Centered Medical Home (PCMH), Diabetes Recognition (DRP). Patient Centered Connected Care (PCCC), and Health Equity.
  • Able to identify discrepancies in quality related to program requirements and propose and implement improvement plans.
  • Maintains account knowledge within NCQA portals, and able to generate invoices and secure needed publications and resources.
  • Demonstrates understanding of the core concept areas of PCMH recognition: team-based care and practice organization, knowing and managing patients, patient-centered access and continuity, care management and support, care coordination and care transitions, and performance and quality improvement.
  • Able to extrapolate and manage data when required from key Epic sources (e.g., QlikView, Dashboards, etc.).
  • High degree of professionalism and capacity to manage relationships with a wide variety of individuals in differing roles, e.g., Providers, clinical staff, Operations leaders, senior management, Board members.
  • Ability to prioritize and manage multiple tasks concurrently, excellent self-management and organizational skills, and ability to work as a team-player.
  • Excellent written and oral communication skills with high degree of competency in manipulating and presenting large data formats.
  • Excellent computer skills including MS Office suite and experience with advanced software (e.g., Visio) preferred.
  • Good understanding of performance improvement management methods (e.g., Lean Six Sigma).
  • Ability to be flexible and manage time and multiple tasks with minimal supervision.
  • Must be willing to work irregular/extra hours and travel in-state as necessary. Provide timely response to requests and deadlines. Receive, process, and provide visual and verbal information.
  • Must be an excellent team player and have a proven history of positive improvements within a collaborative work environment.
  • Strong administrative and organizational skills with excellent attention to detail.

Essential Tasks And Responsibilities

  • Communicates clearly a strong understanding of population health strategies, goals, tools and programs that improve patient health outcomes, health equity, and clinical practice efficiencies.
  • Implements, maintains, and serves as a subject matter expert for NCQA recognition programs for NGPG ambulatory care clinics.
  • Maintains evidence and documentation requirements for all recognition programs.
  • Coordinates all aspects of coaching and engaging the NGPG learning community for successful recognition, including developing and managing the NCQA recognition program education and communication plans; organizing and supporting transformation events (learning sessions and webinars) and special series webinars; developing and deploying educational and communications tools (i.e., such as a e-Newsletter); and other creative methods.
  • Collaborates with key stakeholders in the development and maintenance of a population health management website/intranet plan, including timing of updates and maintenance.
  • Works with the Director of Population Health to develop and modify policies relative to recognition and population health initiatives.
  • Coordinates the collection, analysis, and reporting of NCQA program data through the supervision of the Director of Population Health, engages and coaches NGPG operational/clinical staff and providers in breakthrough strategies, providing communication, education, and support.
  • Develops communication and educational tasks as appropriate (e.g., email distribution of webinar reminders and e-newsletters).
  • Collaborates with the Director of Population Health and NGPG senior leadership to determine reasonable recognition timetables.
  • Provides educational and communications strategies and tools to provider groups and physician practices targeted for recognition; builds and maintains excellent relationships with participating practice contacts in order to advance education and communication goals.
  • Collects relevant data and background information needed for the development of project work plans; works with practices to adhere to timeline and meet reporting deadlines to ensure timely completion of program tasks. Collaborates with other affiliates on population health initiatives and programs (e.g., HP2, NGMC).
  • Provides education and communications support to project teams, workgroups, task forces, learning sessions, and other meetings as necessary, including preparation of meeting agendas and materials, facilitating meetings, preparing meeting minutes, and ensuring appropriate follow up of action items.
  • Assists with development of project surveys, contracts for needed services, project materials and communications as needed.
  • Coordinates the collection, management of project data, and project summary reports with guidance from the Director of Clinical Quality including the PCMH recognition portal (Q-Pass).
  • Collaborates with the Clinical Quality team in identifying opportunities for closing care gaps and ensuring workflows are compliant with recognition program requirements.
  • Prepares and shares routine reports on population health program activities and initiatives.
  • Identifies new opportunities for improving the health of NGPG\’s populations.
  • Ensures collaboration with public relations to develop marketing for recognition programs, and patient education, and maintains knowledge of program-related governmental and regulatory requirements.
