Licensed Practical Nurse -LPN -Middleton – Per Diem

Shift: 11 pm-9 am Per Diem

Job Summary:
Provides skilled nursing interventions to individuals living in a residential setting. Assesses and identifies any health-related needs, monitors medical status and reports changes to the on-call Registered Nurse (RN). Works with residential staff to support the individuals in their community residence.

Primary Responsibilities:

• Understand the clinical and social needs of the individuals\’ receiving services.
• Serve as a liaison between the community and the home.
• Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care.
• Assess individual throughout the shift to determine nursing needs. Report to and seek direction from the RN Case Manager or on-call RN as needed.
• Present information regarding any individual in a professional manner in all settings including meetings, phone contact, daily written documentation and /or email correspondences.
• Responsible to assist at physician\’s appointments, community trips, and other activities as assigned.
• Prepare and teach health related in service to staff and provide ongoing supervision of ADL and other health related routines of each individual.
• Maintain a license in good standing and certifications as required by the program, the funding source, and OSHA including but not limited to provider-level CPR, Universal Precautions, and Human Rights.
• Provide 1:1 supervision of the individual during transportation; and during Day Program participate with other individuals and implement Day Habilitation Goals.

Residential:

• Utilizes a PBS approach when working with staff and individuals, including promoting the agency objectives related to personal growth, community involvement and decision making .
• Facilitates meaningful community experiences for everyone that fosters skill acquisition, relationship building and independence.
• Participates in supervision, general staff meetings, and trainings as scheduled.

Requirements:

• Licensed LPN in MASS with minimum 1 year experience
• Experience with multiply handicapped/developmentally disabled individuals a plus
• Must have a valid Driver’s license and proof of a good driving record.

Pay Rate: $ 36.00/hr.
