About the role

Are you passionate about working with people to promote independence and dedicated to making a difference to people’s life? We would love to hear from you! We are looking for an enthusiastic and dynamic personto join the Kettering Adult Social Care Community Teamin North Northants Council (NNC).

You will be responsible for managing a caseload of individuals; assessing social care needs; developing, co-ordinating and reviewing care support, as well as ensuring that we meet our duty to safeguard vulnerable individuals. You will be committed to working with customers in a person centred way to support them to achieve effective outcomes.

You will be supporting people within the community to be able to remain as independent as possible within their own home and reduce the risk of crisis support. Tasks may include but not limited to assessment and provision of low level mobility aids, assisting with home management tasks, provision of items to meet basic needs such as emergency food parcels, supporting individuals with housing, finances, medication and follow up appointments.

We are looking for people who believe it is important to ensure people have a choice and control over their own support needs. We want people who are committed, like us, to focus on the person, their strengths and networks rather than routinely assessing people for services and eligibility. The three conversation model paving a new way of working in adult social care and we are looking for workers who are enthusiastic and passionate to work with us on this journey.

You need to have good organisational skills, and will be an effective communicator used to working in conjunction with a range of other organisations, including health and other professional colleagues. You will also have a strong demonstrable commitment to working from a strengths based perspective.
